Carbohydrates are rarely converted to fat (a process called de novo lipogenesis) under normal dietary conditions. There are exceptions when this occurs. One is with massive chronic overfeeding of carbs. I’m talking 700-900 grams of carbs per day for multiple days. Under those conditions, carbs max out glycogen stores, are in excess of total daily energy requirements and you see the conversion of carbohydrate to fat for storage. But this is not a normal dietary situation for most people.
Based on those studies, de novo lipogenesis (DNL) still seems to be rare, at least in a fairly well-balanced diet among insulin sensitive people. You discussed one of those rare circumstances in The Ketogenic Diet -- people who already had full glycogen stores who were eating excessive amounts of carbs. The studies I came across (references below) mentioned some other circumstances that did not involve overfeeding:
- In lean or obese people (male & female) eating very low fat/high carb diets (fats 10%, carbs 75% of calories) that are high in sugars (Hudgins 2000).
- In hyperinsulemic obese men even when eating high fat diet (40% fats, 46% carbs) (Schwarz et al 2003).
- In hyperinsulemic obese and normoinsulemic lean or obese men on a low fat/high carb diet (20% fats, 68% carbs; of total carbs, 54% were simple sugars, 46% complex carbs) (Schwarz et al 2003).
- (I couldn't find a study of women comparable to Schwarz et al 2003, but the study by McDevitt et al. 2001 of overfeeding women seems to be comparable to studies of overfeeding men.)
